If no ID is passed, creates a new unique id based on web and topic.
Reads a setting from the FOSWIKIPREF cookie.
Returns: - 1 if the cookie has been set (meaning: show content)
- 0 if the cookie is '0' (meaning: hide content)
- undef if no cookie has been set
